i am being bullied at work and it is just so unorganised and dirty there, what can i do to get them reported?

ok so i started working at this dental practice in manchester just under 2 months ago. from day one i noticed it was breaking some health and safety and cross infection policies. now ive been there a bit longer and gone to different websites, i have found out that she CAN get shut down because it is realy bad there.
also, i am getting badly bullied there, by the main dentist who also happens to be the boss! she and another dental nurse (who happens to be her best mate) are always niggling at me, talking about me at home etc and everytime they start on me its always in front of the patient/s…obviously to make her look big and hard. exept the patients do notice and often ask me if im ok…well it wouldnt shock you to say that half the patients don’t come back again!!! i reckon this is racial discrimination as i am the only white girl there…and the other dental nurses in the past (all white) have left for the same reasons! but i dont want to leave as i thoroughly enjoy my job and always have!
also, the place is badly run. i havent had a contract, either verbally nor written! and when i got my payslip last month, the figures were wrong, so she took it back off me and hasnt given me a new one. i am supposed to get paid on the 10th of every month, except i havent this month and now i am struggling for the rent and food.
i recently recovered from a bad spell of depression (being only 21)…and now it is coming back and is playing with my relationship! i feel so down again and just cry whenever i get the chance. but my boss wont let me have any time off and she wouldnt let me go home one day when i was ill, so i keep failing my doctors appts!
if anyone can guide me in the right direction as to what i can do about all this, i would be more than gratefull.

Find another job, equivalent to this one, then move!
all that information you gathered from the websites you went too…. use it!!!! but first find a new job, than quiet that one
than use that information to report every single detail you have noticed. don’t get fired but rather quiet, but have a job waiting when you do. do this smart. some things are planed for a whole year to do perfectly. sit down and use your time wisely. job search and put in resumes and wait 
play cool at your job cause they are hard to come by 
but it is possible to get another one 
stay positive no matter what it takes ( what ever it takes!) play cards, go to a park, read some jokes, talk to new people and no matter what the out come, learn from it and be happy. life is too short and that stuff is not worth it. your life is worth more than you can see
